<p align="center"><strong>APPLICATION FOR CONSIDERATION</strong></p>
<p><strong>PURPOSE:</strong></p>
<p>This Fund was established to provide partial scholarships to members of CBTU Ontario Chapter, their children, grandchildren or family members or youth in the Black community pursuing a post-secondary education.</p>
<p><strong>ELIGIBLITY:</strong></p>
<p>Any student who has graduated or will be graduating from an Ontario High School or an equivalent program and who will be or is attending a recognized post-secondary institution such as a college or university, and is a member, child or grandchild of a CBTU member in good standing, or a family member or youth in the Black community, is eligible to apply for consideration.</p>
<p><strong>AWARD:</strong></p>
<p>One scholarship will be awarded on an annual basis to the successful applicant as chosen by the Fund’s Selection Committee. The scholarship will be $1,000. In the event that two applicants are equally qualified, two scholarships will be awarded at $500.00 each.</p>
<p><strong>CRITERIA:</strong></p>
<p>The Fund’s Selection Committee will review all applications received for consideration. Scholarships will be awarded at the discretion of the Fund’s Selection Committee which will consider the following criteria:</p>
<ul>
<li>Essay submission – 500 words. Topic: Why is there a need for an organization like CBTU and how will you as a recipient support the organization’s mission?</li>
<li>Involvement in community activities including: multi-cultural, religious, athletic and any other organizations</li>
<li>Paid or volunteer work experience</li>
<li>Secondary school marks and academic achievement</li>
<li>Financial need</li>
</ul>
<p><strong>INSTRUCTIONS:</strong></p>
<p>1. Fill in all information on the Application for Consideration. If more space is required, please attach a separate sheet. Any missing information may result in disqualification of the application. Once applications are submitted, applicants will not be contacted unless successful.</p>
<p>2. Answer completely the essay portion, typed and double-spaced on the questions on page 5 of the application. Separate sheets may be used.</p>
<p>3. Attach a certified academic transcription from your secondary school and any other relevant documentation.</p>
<p>4. If selected, a passport size photograph will be required.</p>
<p>5. <strong>DEADLINE FOR SUBMISSION is October 1 OF EACH YEAR.</strong></p>
<p>Submissions should be couriered, hand delivered or mailed in a sealed envelope to:</p>
